Family Night is an opportunity to introduce residents and family members to Pharmore. Most people outside of assisted living, skilled nursing, supportive living, memory care, or behavioral health facilities do not fully understand the role of a pharmacy. From improving care to ensuring appropriate billing, during Family Night, we cover the fundamentals. We also provide guidance on selecting and enrolling in the best Medicare Part D plan for each resident and we make our team available for a private consultation to discuss medication(s) or other clinical issues, if needed.
While going home from an assisted living, skilled nursing, supportive living, memory care, or behavioral health facility is an exciting transition for residents, managing prescribed medications and dosages can be confusing. Pharmore’s Face-to-Face program ensures a successful transition home by assigning a dedicated Pharmore consultant pharmacist to residents to explain their medication regimen, including medication name and class, indications, dose and directions, and potential side effects. The Face-to-Face program also incorporates research on the way older adults learn, ensuring our team shares information succinctly and utilizes well-organized prescription labels.
Strategies for Reducing Avoidable Readmissions in Long-Term Care Facilities
The number don't lie. A reported 75 percent of readmissions – or over 4 million — are preventable, with $1 of every $10 spent on healthcare – or over $30 billion – tied to avoidable readmissions.
Carline Joseph, PharmD, Consultant Pharmacist at Pharmore, is actively addressing preventable hospital readmissions by promoting resident education and effective communication between residents and pharmacists.
